Implementation of the Braden Scale with specific interventions to improve pressure ulcer prevention across a three hospital system

Three hospitals within the same hospital system utilized a pressure ulcer prevention care path; however,the tools to predict the development of pressure ulcers and the interventions differed. In an effort to improve pressure ulcer prevention across the system,simplify education of staff and make is easier for staff to provide appropriate care for the patient at risk for skin breakdown,the Braden Scale with interventions was implemented. The Braden Scale for predicting pressure sore risk is a research based tool. We developed specific interventions not only for each subscale but also according to the level of severity within the subscale. This poster presentation will describe the process involved in this effort from the beginning planning meetings and staff education through current efforts to assess staff competence and improvement in pressure ulcer prevalence. It will include successes and stumbling blocks encountered. We have decreased our pressure ulcer prevalence rate and improved skin care for our patients through this process. Note: Interventions related to skin care were simplified and cost was decreased by utilizing products from one specific vendor. This vendor did assist in staff education and sponsored a poster that was distributed to each hospital unit.
