4425 Choice of Treatment Methods of Ostomy Complications

Complications (both stoma complications and peristomal skin disorders) developed after ostomy surgeries present a difficult problem for many people with a stoma. Overall complication rates following stoma formation of between 21 to 70 %. Variety of clinical, topographic and anatomical forms of ostomy complications makes interpretation of data, choice of treatment very variable and ambiguous.
            The aim of the study is to determine treatment methods of ostomy complications. Indications for surgical or conservative treatment are assessed according to the developed algorithm, depending on the type of complications and contraindications.
            In the period from January’05 till December’08 2033 people with a stoma were registered in the Ostomy Rehabilitation Centre of the State Scientific Centre of Coloproctology. To identify treatment methods for 788 people with ostomy complications the corresponding algorithm was used. 255 patients were diagnosed for surgical treatment including: 148 with stoma complications and 107 with combined complications. 533 people with a stoma including 288 with peristomal skin disorders, 77 with stoma complications and 168 patients with combined complications received conservative treatment because surgery was impossible due to severe concomitant or underlying diseases. Effectiveness of treatment was estimated on the base of clinical assessment of the results and safety of treatment.
            To assess the participant’s satisfaction with medical results of treatment special questionnaires (Stoma-QoL) was analyzed before and after treatment.

            Timely, effective high quality treatment can improve quality of life of people with a stoma. Choice of treatment methods depends on the type of complication, its severity and contraindications.
