Abstract: There are over 2 million patients per year suffering from venous ulcers. These wounds are costly to treat and are a vast source of pain and suffering to the patient. Chronic wounds pose clinical challenges within the field of wound care in the inpatient, outpatient and home care setting. Providing seamless treatment is essential in managing this growing patient population. Our objective is to identify clinical pathways for venous ulcer clients treated inpatient, transferred to home care, and provided medical support by a Nurse Practitioner (NP)-driven wound care clinic. This presentation focuses on protocols used in venous ulcers. Through a triad of effective collaboration between the NP, inpatient and home care Certified Wound Ostomy Continence Nurses (CWOCN), an encouraging model toward assessing, treating, and healing difficult wounds has begun. An overview of the pathology, nature of chronic wounds and their management are presented. The NP-practice model clinic is one of a few in the United States. A wound assessment to assist the NP in the primary care arena is included. The CWOCN focuses on the challenges of working with patients with venous ulcers in the home care setting and provide examples of the support required from the primary care physician (PCP).. Documentation for venous ulcers on the Outcomes Assessment Information Set (OASIS) is included as well as long term expectation for patients in the community.
