Madonna Rehabilitation Hospital (MRH) Ostomy Product Matrix: Assisting Nurses and Personnel in Purchasing Department in the Selection and Provision of Ostomy Appliance

Purpose: To assist the nurse in selecting an ostomy product for patient use from the MRH Purchasing Department. Objectives: Increase nurses product selection knowledge, comfort and competency with the use of available products thru the “Pair System.” This system refers to the grouping of ostomy pouch and barrier/ wafer which aids in communication between the nurse and personnel from the Purchasing Department. Problem: Our facility is composed of 245 beds with LTACH, Acute Rehab, Ventilator Assist Unit, Extended Care Units, Alzheimer's Unit and Assisted Living with patients with various needs for ostomy appliances. Purchasing Department personnel requires product codes from staff nurses to be provided to them every time a request is made for an appliance. Due to lack of product knowledge and lack of competency in product selection, nurses continued to requests for ostomy appliances from various manufacturers per supplies that comes in with every patient that is being admitted. Build-up of supplies and complaints accumulated in the Purchasing Department since some patients are discharged even before supplies come in. Solution: The Ostomy Product Matrix has been updated. Nursing staff refers to the pair or group number of specific ostomy supply being requested according to nurses assessment of ostomy type, stoma size and presentation. If a patient has colostomy, stoma size of 2” with protruding stoma, nurse will request for Pair#1, eliminating need to obtain a product code for the stoma wafer and pouch. Policies and procedures updated, copies of Ostomy Product Matrix in 5 ½” x 8 ½” laminated cards distributed to each unit, and product inservice has been provided by ostomy nurse and manufacturer's representative. Outcomes: Complaints has been eliminated with improved communication between Purchasing Department and nurses. Nurses became more competent and knowledgeable in choosing the appropriate ostomy appliance for their patients.
