Skin Care and the Long Term Care Nurse: From Novice to Expert

Purpose: A "multi-module" education program was developed to educate long-term care nurses about preventative skin care, assessment,treatment, and documentation in response to the new CMS guidelines for wound and pressure ulcer care. Objective: A CWOCN developed education program provides nurses in the long-term care setting with an increased knowledge base to care for residents at risk for and with wounds/pressure ulcers. This multi-demensional program consists of classroon instruction, independent study, and hands-on competency evaluation. The program is designed to aid these nurses in recognizing residents at risk for wounds/pressure ulcers not only based upon a risk assessment instrument but in using information from the medical history and physical assessment. Additionally, these nurses are empowered to interact within the interdisciplinary team to provide interventions for prvention and treatment and the promotion of healthy skin. Residents that are admitted with or develop wounds/pressure ulcers can be assured accurate assessment and documentation by the nurse that can aid the physician and interdisciplinary team in making appropriate treatment decisions to promote healing. Understanding the rationale for treatment allows the nurse to effectively communicate the goals of wound healing to the resident and family. Once the wound is healed nurses can again identify risk factors and initiate interventions to prevent reoccurrence. Outcome: This program will increase the long-term care nurses ability to understand different types of skin injury, preventative interventions and treatment options to improve the quality of care to their residents.
