Chronic Leg Ulcer Healed in only Seven Dressing Changes Using Silver Polymeric Membrane Dressings*

28-year-old male had a continuously painful yellow right lower leg ulcer 4.5cm x3.7cm, depth 0.3cm. Previously treated over 9 months with sharp and enzymatic debridements, wet-to-dry dressings, topical antimicrobials and oral antibiotics based upon cultures, all with little improvement. Co-morbidities: untreated Crohn's disease, venous hypertension. Smoked 2 packs/day. Patient's employment required long periods of standing.

Polymeric membrane dressings save time because no wound-bed cleansing or peri-wound skin protection is needed. Dressings are absorbant and non-adherent, which increases comfort. Silver polymeric membrane dressings have additional antimicrobial properties.

Silver polymeric membrane dressing applied, covered with compression wrap at 30-40mm Hg. Dressing remained in place for 7 days without maceration. Dressing changed weekly until wound closure. Polymeric membrane dressing able to deliver needed wound care functions in one easy–to-use dressing. Silver polymeric membrane dressings were utilized to closure because of patient's history of infection.

Wound became cleaner and pinker, with very little fibrin build-up, in contrast to previous utilized antibacterial cream or even topical debriding ointment. Intact lower leg skin was washed with mild soap and moisturized under compression wrap to maintain healthy skin integrity. Care taken to not wash, irrigate or disrupt wound bed.

Patient's pain decreased and compression therapy was tolerated. The ulcer healed without maceration within 6 weeks of initiation of silver polymeric membrane dressings using a total of seven 4x4 silver dressings.

Silver polymeric membrane dressings were used with compression therapy for successful management/healing of this venous hypertension wound. Dressings' reduction in patient's wound pain allowed patient to tolerate clinically therapeutic compression. Silver polymeric membrane dressings were able to deliver all needed functions in one easy–to-use dressing.
