Implementation and Evaluation of an Advanced Practice Continence Nursing Role in an Acute Care Hospital

Urinary incontinence is both prevalent and poorly managed in acute care hospitals. Evidence suggests that a continence nurse specialist role can have positive outcomes on quality of life for patients and reduce costs for organizations as well as contribute to improvement in the continence practice of generalist nurses. The Wound, Ostomy, Continence (WOC) nurse is uniquely prepared to provide comprehensive assessment and treatment of urinary and fecal incontinence to reestablish continence or to improve management of incontinence and quality of life. Additionally, the advanced WOC nursing role can involve assessment, planning and implementation of strategic organizational initiatives to address issues such as product utilization, urinary tract infections, or overuse of indwelling catheters. This poster will focus on the implementation and evaluation of an advanced practice nursing role within an acute care hospital in Ontario, Canada. This Clinical Nurse Specialist-Continence role involves assessment and treatment of hospitalized and community dwelling patients in a continence clinic setting as well as endeavoring to positively influence continence care by staff nurses. The role development based on the Strong Model of Advanced Nursing Practice will be reviewed. Role evaluation strategies such as incontinence specific quality of life and catheterization incidence will be analyzed. Key patient care issues identified and addressed within the first year such as inappropriate use of indwelling catheters will be discussed. The poster will also address the pivotal relationship evolving between WOC nurses in acute care and those in the community in creating a continuum of care for patients experiencing urinary or fecal incontinence.
