Do products affect wound outcomes when using standardized algorithms?

Introduction. Protocols based on a content validated1,2 algorithm3* of wound care guided by valid patient risk and wound assessment scales reduce pressure ulcer incidence4 and improve acute and chronic4-6 wound outcomes while reducing costs of care.4-6 The value of rigorously adhering to a product formulary when using such a protocol is often questioned. Purpose. Summarize effects on wound care outcomes achieved using or not using specified product formularies within standardized protocols of care derived from a content validated wound care algorithm3. Method. A systematic review was conducted of clinical studies published using standardized wound care protocols based on a content-validated algorithm3. Reported outcomes for subjects managed using the specified protocol formulary were compared with those obtained using products not normally on-formulary. Results. Five studies2, 4-7 of more than 1310 chronic and 158 acute wounds, were identified in the literature search. Using the evidence-based product formulary* was associated with faster pressure ulcer healing7. A prospective comparative study6 reported improved pressure ulcer status as measured by PSST (Pressure Sore Status Tool) scores (p=0.046) and reduced costs of care (p<0.005) in pressure ulcers managed with primary dressings normally specified in the standardized protocol compared to using the same protocol with gauze-based primary dressings. Conclusions. Using evidence-based products within standardized protocols derived better, more economical wound outcomes than using off-protocol wound care products within the same protocols of care.
