Statement of Problem:  Changes in the October 2008 Center for Medicare/Medicaid services on re-imbursement finally recognized the importance of prevention. The key elements in this reform are prevention, education, and accountability of patients and healthcare providers.  Since skin is the largest organ in the body and the first line of defense against infection, skin hygiene must be a priority.  Published literature documents assessment of the wound and surrounding tissue cannot be performed until the skin can be viewed for complete evaluation.

Description of Past Management:  This Wound Clinic previously presented and initiated a skin care program using concentrated melting moisturizing creme to lubricate, protect, and heal skin and wounds.  It was reported that concentrated melting moisturizing creme enhanced this process by facilitating the removal of callous formations, scabs, other wound product residue, and dry flakey skin without causing tissue trauma.  The regime was considered easy to use by all.

Currenat Clinical Approach:  The purpose of these multiple case studies is to review 10 years of evidence-based data from this Clinic's current skin hygiene regime for evidence that concentrated melting moisturizing creme effectively facilitates skin hygiene and wound visualization without tissue trauma.

Outcome:  A review consisting of over 2,000 clients/year supports the continued use of concentrated melting moisturizing creme when used per protocol.  Photographic data showing a variety of clients before/after application and follow-up weeks and months later will be shown.  The proper application and removal techniques will be depicted.

Conclusion:  The study results reinforced the importance of wound visualizaatin and proper skin hygiene.  Use of concentrated melting moisturizing creme did not traumatize the skin and avails the healthcare provider with tools to properly assess and treat the wound.  Our current regime supports the benefits of prevention, education, and accountability for patients and healthcare providers resulting in improved skin hygiene.
