Abstract Title : District Skin Care Team "Reduces Pressure Ulcer Prevalence" District Wide
Purpose: Would a skin care team, composed of RN's and LVN'S from different pavillions in a teaching hospital district, be able to work together to create skin care protocols and implement at the unit level.
Objectives:
            Attendee will be able to identify key concepts to develop a successful and productive skin care team.
            Attendee will be able to identify key concepts to maintaininga successful and productive skin care team.
Abstract:
            Problem:  Stage III and IV Pressure Ulcers were developing at the Critical Care level for Trauma patients District Wide 
            Rationale:  Pressure ulcers were developing because a skin care protocol had not been established or implemented.
            Methods:  Skin care team member representation from each unit and pavillion met monthly for 3 hours.  Education regarding skin, wounds,
                            products, documentation were discussed during education hours.  Protocols were developed and implemented by the skin care champions. 
                            The skin care champions function as the "resource" for the unit and have the ability to assist with decision making regarding skin care. 
            Results:    Improvement was measured by Prevalence and after 6 months, a significant reduction was seen.  Prior to the skin care champion team,                                       Prevalence of Pressure Uclers District wide estimated at 15%, and currently District numbers are below National levels at 7%.
            Conclusion:  In conclusion, it is imperative to have skin care representation at the unit level to assist with implementation of protocols, and maintain awareness  
                             of importance for pressure ulcer prevention.
