Effects Analysis of Wound Care of 41 Full Thickness Wounds Complicated to Disseminated Intravascular Coagulation Followed by Severe Epidemic Encephalitis

Abstract Background and Objective Full thickness wounds complicated to disseminated intravascular coagulation are easy to bleeding and being affected, thus they are difficult to healing. From April 15 in 2008 to June 15 in the same year, according to one three years old child with 41 full thickness wounds which total size is about 452 cm² complicated to disseminated intravascular coagulation followed by severe epidemic encephalitis, we study on stages interventions that can not only decrease stress and injuries reaction, but also enhance wounds healing. Methods Early stage interventions are focus on protecting wounds and skin from exudates ,sweat and other body fluid in order to  preventing wounds from affection during critical care. Moderation stage interventions: after discharge ,41 wounds are randomly divided into two groups, 21 wounds and 20 wounds,especially. Two group wounds are debrided by autolysis combining with mechanical debridement and autolysis combining with conservation sharp debridement, especially. After debridement, hydrocolloid dressing and foam dressing is used until wounds healed. Healed stage interventions: From healed seven days to six months, prevention scars treatment used by occlusive silicon dressing and followed-up once 3 months until 12 months. Observation indications: debridement duration and healing time of each wound. Scars scores and activity ability scores are recorded during followed-up. Statistics analysis used by SPSS 11.0.Results 41 wounds is completely healed. Average healing time is 26.49±7.47days and Average debridement duration is 7.83±5.37days. Average debridement duration of two groups is 9.90±5.82dVs5.65±3.91days, P<0.05. Average healing time of two groups is 27.81±8.33dvs25.10±6.35days, P>0.05.Scars scores and activity ability scores at followed-up 6 months are more than ones at 3 months(P<0.001).Scores at followed-up 9 and 12 months are more than ones at 6 months(P<0.05 and 0.001).Conclusion Stages interventions enhanced wounds healing and prevention scars treatment must be continuously 6 months that can get solid effects.
