Abstract:A patient with diverticulitis with perforation underwent ileostomy in emergent situation. After the operation, he was found to have dehiscence between the ileostomy and the surrounding skin. From January 22 to April 2, 2009, we conducted an overall assessment using the Roy’s adaptation model. With physical assessment, observation, and interviews, the patient was confirmed to have "impaired skin integrity," "nutrition less than body needs "and" anxiety "and other health problems. We assisted the patient in physiological adaptation, self-concept, role function, and interdependence of all aspects of adjustment, especially for the issue of ileostomy leakage and the surrounding wound erosions. We provided photographs and methods of wound care, to achieve consistency and continuity of nursing care. The stoma speci was also consulted to provide the skill, expertise of wound care, and the support system, in order to relieve the patient and the family’s anxiety. The patient could face the treatment frankly for achieving the integrity of care. We wound like to share the experience of “enterostomy and skin dehiscence” with everybody.
