    This poster contains information about a specfic patient problem related to a new urostomy and how the WOCN addressed the problem with a practical solution of an individualized ostomy instructional booklet. The poster discusses how a patient-specific problem and solution was developed into a hospital-wide initiative. It includes specific information, as well as pictures, of the assessment, development, implementation, and evaluation process of developing ostomy instructional booklets for staff and patient use throughout the organization. Through this poster, the participant will be able to identify a patient need and discover ways to address that need in a practical way that can be applied to other patients. The participant will also be able to use the specific information provided in this poster to understand the educational issues ostomy patients face.
