This abstract outlines the development and implementation of an inpatient, acute care Skin & Wound Assessment & Treatment team. The poster will describe rationale and goals, champion selection, training and resources, utilization, continuous education and outcomes.  Algorithm creation and utilization for Pressure Ulcer Prevention was also included in SWAT team implementation. These were evidence based and will be included in the poster presentation.  Road blocks/gaps, future interventions and considerations will be discussed.  How the hospital related pressure ulcer and related skin breakdown outcomes were impacted, will also be included.
