Purpose: This case study examines and demonstrates how data tracking changed strategic planning and influenced practice model changes in a physician dabbler/nurse driven Wound Ostomy Continence outpatient wound clinic. This case study will show the importance of implementing a physician/mid-level provider practice model into your wound care setting to improve clinical outcomes. Through the utilization of research based advanced wound care modalities and treatment algorithms, data indicates improved healing rates, improved financial outcomes and increased service opportunities utilizing a full time physician/mid-level WOCN provider model. This case study will highlight the benefits of provider collaborative practice opportunities and the benefits of advanced wound care modalities. This case study will also identify strengths and challenges of transitioning from a physician dabbler/nurse driven WOCN model to a full time physician/mid-level WOCN provider model in our clinic. There are opportunities available to mid-level WOCN providers to develop successful autonomous practices in critical access and rural community hospital settings when provided with the appropriate collaborative practice structure. This case study identifies such a practice model and demonstrates that a hospital based wound care program in a small community hospital can be successful and have market penetration in a competitive wound care environment.
