INTRODUCTION: A quality improvement (QI) intervention in PU prevention was conducted with the purpose of reducing the prevalence of hospital-acquired pressure ulcers (HAPU) through practice innovation efforts designed to create a clinical culture of HAPU prevention. METHODS: The multifaceted QI efforts spanned from 2008 through 2010 utilizing Plan-Do-Check-Act methodology. The objectives included 1) adopting an interdisciplinary team approach to HAPU prevention; 2) updating the Skin Care Policy using evidence-based guidelines; 4) use of change management to ensure caregiver adherence with best practices; 5) updating support surfaces; 6) mandatory caregiver education on appropriate risk assessment, documentation, facility policy, and application of evidence-based best practices in HAPU prevention; and 7) integration of technology to enhance caregiver adherence to HAPU prevention. The effectiveness of the QI initiative and objectives were measured through ongoing and annual prevalence audits and monitoring caregiver adherence with facility policy and best practices. RESULTS: The QI initiative resulted in a 63% reduction in HAPUs between 2008 and end-2010. CONCLUSION: Ongoing interdisciplinary and multifaceted QI improvement efforts can be successful in improving patient safety and reducing HAPUs, and should focus on ensuring best practices are provided at the patient’s bedside.
