6014 Research on quality of life and Influencing factors in colostomy patients with rectal cancer in Xi'an

Abstract
  • Background and aims:  To understand the quality of life(QOL)and influencing factors of colostomy patients with rectal cancer in Xi’an , formulate standard stoma care measures,so as to provide evidences for promoting the development of stoma specialist care and improving the QOL of permanent colostomy patients.
  • Methods: 145 cases of colostomy patients with rectal cancer in Xi’an were retrospective surveyed through EORTC-QLQ-C30 and a tumor-specific module(QLQ-CR38), and the data were collected for statistical analysis.
  • Results: The average scores in overall health/QOL of colostomy patients were 60.31±23.19, The scores of role function and social function were significantly lower than the reference value(P<0.01), the scores of diarrhea and economic difficulties were significantly higher than the reference value(P<0.01),The sexual function scales is the lowest score of QLQ -CR38 ,The male sexual problem scores highest in the symptom scales,which followed by is the female sexual problems, stoma-related issues; Multiple stepwise regression analysis showed the psychological condition, postoperative time, complication were into the regression equation of factors,and the psychological condition and postoperative time were positively correlated with the QOL,the stoma complication was negatively with the QOL.The complication incidence rate was 40.7%, Logistic regression analysis showed that inappropriate stoma location, inappropriate defecation and unacceptable telephone follow-up were the major risk factors of complication.
  • Conclusion :The sexual function,role function and social function of colostomy patients were significantly affected, and the diarrhea symptom and economic difficulties were severe. To improve the QOL of colostomy patients, Specialist stoma nurses should strengthen psychological nursing for patients, training in patients with regular bowel movement and instruct their to correct understand, prevente and treat stoma complications through variety of ways.
