Because the patient suffered from infection and convulsion symptoms, she received treatment of Phenytoin and Piperacillin/Tazobactam After the treatment, the patient had blister all over her body and oral mucosa. During hospitalization period, the mucous membranes ulceration began to appear on her body and more than 10%.The patient’s problems were identified as impaired skin integrity, body image disturbance and pain. Nursing care focused on prevention of infection and increasing the patient’s confidence. According to the patient’s problems, health care team offered individual care including providing adequate wound dressing, positive mental support and pain relief. We not only took care of the wound, but also encouraged families to participate in the care process. On the wound healing process, we taught patient and families to massage and use ice packing to reduce the sensation of discomfort. During the nursing process, patient was willing to share her feeling and accept the change of body image. After a series of professional nursing intervention, the patient obtained complete recovery . As a result, the patient had no infection sign and any complication. After the patient’s skin condition remained stable and intact, she was discharged safely from the hospital.
