Evaluation of the efficacy and necessary characteristics of decubitus ulcer prevention measures using electronic nursing record

[Purpose]Currently, numerous IT applications, such as an electronic nursing record are being developed in the field of nursing in Japan. In order to evaluate the efficacy of decubitus ulcer prevention, as well as identifying the future requirements for improving the quality of prevention efforts, we investigated the procedures for decubitus ulcer prevention using these electronic nursing records. [Method] An action was conducted in the form of using a project management method. Subjects of the survey included 1537 patients hospitalized between April and May 2004. Information was extracted from the treatment plans and nursing support systems in IT chart that were developed for decubitus ulcers in 266 of these patients. The survey contained 84 categories related to patient characteristics. Statistical analysis was performed using SPSS version 12. [Results] It was found that 266 out of 1537 patients at risk from developing decubitus ulcers (17.3%) were selected using the decubitus ulcer prevention process, and success rates for prevention exceeded 90% when preventive care measures were employed. The percentage of prevention among hospitalized patients was in excess of 98%. The analysis of care plans classifying the effects associated with the elimination of pressure and friction, improved skincare, improvement of nutrition, and rehabilitation for mobility function all exhibited low correlations between the treatment plans and the results of the survey. Descriptions of nursing performance in IT records revealed that the frequency of skincare exceeded that of attention to other risk factors, while the factors of pressure alleviation and friction, nutrition improvement and rehabilitation for mobility function were performed less frequently. [Conclusion] IT chart was useful for collecting data to identify the issues of decubitus ulcer care. In addition, exchanging of information among rehabilitation teams and WOC nurses regarding the assessment is important to improve the quality of decubitus ulcer prevention and care.
